Registration
At Canyon Vista Medical Center, you are our top priority. Our main concern is your care and well being. That’s why our Admitting & Registration department’s main focus is to get you registered as quickly and smoothly as possible.
When you arrive at our hospital, the Concierge will take some basic information and then will guide you to a registration window. During registration, the Admissions Clerk will verify current information, your appointment, and double check the physician order. We register patients and perform services based on your physician order. This order must be complete or it may postpone or delay your services.
The information required on your order is:
- Your Name
- Date
- Procedure Being Performed/ordered
- Diagnosis or symptoms (cannot have a “rule out” or “possible”)
- Physician’s Signature
If this information is not available at registration we will need to obtain it prior to registration. Please review these requirements with your physician when the order is being prepared. Your Co-pays or deductibles for service will also be obtained at this time, if applicable. After this process is complete, you’ll be on your way to the appropriate department.
Canyon Vista Medical Center strictly abides by HIPAA laws in maintaining the confidentiality of your healthcare information. We ask that those patients who are waiting to be checked in at Client Services, wait in the lobby area until a window becomes available, to allow privacy and consideration to those patients being processed.
